If you are interested in becoming a new patient with Dr. Smith you will need to call us for a new patient appointment. This appointment will take about 1 hour of your time, to help reduce the time in filling out forms a basic form is included here to get you started. Additional forms, such as HIPPA and Insurance information will be done in the office to ensure your privacy.
Please be 15 minutes early to your appointment to ensure that you can have your questions answered and the forms filled out properly.
